چکیده

شیوه‌ی تامین مالی بخش سلامت از چالش‌های اساسی مسئولان کشورها می‌باشد. در کشورهای در حال توسعه، مانند ایران، بار اصلی تامین مالی بخش سلامت به عهده‌ی خانوارها است؛ این شیوه برای تامین مالی حوزه‌ی سلامت غیرمنصفانه می‌باشد، زیرا در این روش استفاده‌ی افراد از کالاهای سلامت به وضعیت اقتصادی آنان بستگی دارد نه به وضعیت سلامتشان. بنابراین یکی از اهداف مسئولان بهداشتی و درمانی کشورهای مذکور جایگزینی روش پرداخت از جیب با شیوه‌های عادلانه‌تر است. برای ابداع شیوه‌های تامین مالی مناسب در این حوزه ابتدا باید عوامل تعیین‌کننده‌ی پرداخت مستقیم خانوارها شناسایی و تحلیل شوند. لذا این مطالعه عوامل موثر بر مخارج سلامت خانوارهای شهری کشور را بررسی کرده است.
مدل‌سازی مخارج سلامت با استفاده از داده‌‌های طرح هزینه و درآمد خانوارهای شهری و روستایی در سال 1390 - که حاوی اطلاعات 18727 خانوار در مناطق شهری می‌باشد- و مدل‌های انتخاب نمونه انجام شده است، زیرا میزان مخارج سلامت تنها برای زیرمجموعه‌ای از خانوارهای نمونه‌ قابل مشاهده می‌باشد. نتایج برآوردها نشان می‌دهد افزایش درآمد، تحصیلات، نسبت زنان، توسعه‌یافتگی محل سکونت از نظر سلامت و تعداد سالمندان و غیر سالمندان موجب افزایش مخارج سلامت خانوار می‌شود. بهره‌‌مندی از پوشش بیمه‌‌ای و مصرف دخانیات نیز باعث می‌شود خانوار مبالغ بالاتری را در بخش سلامت هزینه نماید. هم‌چنین خانواده‌‌هایی که توسط افراد دارای همسر و هرگز ازدواج نکرده اداره می‌شوند، به ترتیب، دارای بیش‌ترین و کمترین مخارج سلامت می‌باشند. ضمنا، مشکل انتخاب نمونه در بررسی عوامل موثر بر مخارج سلامت خانوارهای شهری ایران وجود دارد و رگرسیون خطی تخمین‌های تورش‌دار و ناسازگار بدست می‌دهد.

The method of financing in the health sector is a principal challenge for authorities. In developing countries, including Iran, the main burden of financing in the health sector is on families. This method is an unjust mechanism because the people’s utilization of health goods and services depends on their economic status, not their health conditions. Therefore, in such countries, one of the goals for health authorities is the replacement of “out of pocket” with more equitable methods. The first step for innovating appropriate financing methods in the health sector is identifying and analyzing the determinants of the ability of families to pay. This paper studies the factors that affect Iranian urban families’ health expenditures.
In this study, health expenditure modeling is done through a sample selection model using the data obtained from the household income-expenditure survey (HIES) for 18727 families in the urban areas. The results show that there is an increase in the household health expenditure, parallel with an increase in per capita income, education, gender ratio, residents’ health development and the number of the elderly and the non-elderly. In addition, insurance and smoking factors cause a higher amount of household spending in the health sector. Also, the families with married members have the highest health expenditure, while those with never-married members have the lowest. Moreover, there is a sample-selection problem for investigating the determinants of health expenditure in Iranian urban areas; the use of linear regression leads to biased and inconsistent estimations.
